Your Guide to the OSAP Forgot Password Form
If you are an Ontario post-secondary student and have forgotten your OSAP password or need to retrieve your OSAP Access Number (OAN), you will need to sign a notarized form in order to reset your password. NotaryPro can help with this form.
The good news is that as of October 2021, the Ontario government recently updated the requirements for completion of the OSAP Website: Forgot Password and/or OSAP Access Number form to allow remote identity validation by authorized third-party officials, such as NotaryPro.
Now you can retrieve your OSAP Access Number (OAN) and/or reset your passwords for the OSAP website by meeting with a NotaryPro rep via simple online video and quickly electronically sign this form, all from the comfort of home.
However if you still have an older version of this form (i.e., one that notes the previous “in-person” requirement, but has been electronically signed and the individual’s identity validated remotely), NotaryPro’s remote online notary service can still be used to validate your identity and electronically sign.
When booking these appointments, you must provide NotaryPro with proof of your Social Insurance Number and government-issued photo ID during the video call (rather than emailed, or uploaded to a website).
